Please read through these instructions carefully as it will guide you as to how to fill out the forms properly

For the LDSS-3370 form, please be sure to list all of the individuals in your household as well as all the addresses of the places in which you resided for the past 28 years. NOTE: There should be no gaps between the years listed for each address.

For the OCFS-4930 form, you will need to compete the form in it’s entirety, print it, sign it and and bring it with you to the DOH fingerprinting appointment.

For the OCFS-6001 form, you will need to pay particular attention to the “Lived in another state” question as you will need to list every address you have resided outside of New York State for the past five years.

For the OCFS-6002 form, in the Education/Training section, you will start with your high school degree and list the other education/trainings you have competed beyond high school.
In the Child Care Experience section, you will list the years of experience for different age groups.
For the Supervisory Experience section, this section should only be filled out if you are being hired for a supervisory position such as Site Director or Assistant Site Director.

For the OCFS-6003 form, Please be sure to list at least on employment reference and one personal reference. These should be the same individuals who will be providing you a reference letter.

For the OCFS-6005 form, please read carefully what if a crime is defined as.
For example: a parking ticket is not a violation, not a crime.

For the OCFS-6022 form, if you do not have a Social Security number, you will need to provide an Alien registration Number.
